During the Summer of 2002, I became involved with the State College area chapter of the American Red Cross. As I became more aware of their organizational processes, I saw the possibility of migrating their donor sign-ups onto the Internet.
This form is what I developed in conjunction with Red Cross representative, Wendi Keeler, to preform this task.
It is scripted in PHP to update blood drive dates, times, and locations on the fly. In addition, it uses JavaScript to ensure that only the proper times are displayed for any particular blood drive.
This Web application also features a customized back-end so that appointment reminders can be sent, drives added and removed, and drive schedules viewed and printed with the click of a button.
Since October 2002, over 4000 blood donors have utilized this online sign-up form.
